What Insurances do you Accept?

Currently accepting all Utah Medicaid insurances and Cash. Private Insurance contracts are pending , so please check back.

How does your cash price work

Cash price is $ 75.00 for Initial visit then $55.00 for follow-up visits. Once you are doing well on a medication that is working, follow-up appointments are required every 3 months.

What is your new patient procedure?

You will be asked to complete a new patient packet and questionaires online. These forms will need to be completed prior to your virtual appointment. You will need to have access to a computer. laptop, or phone to submit forms and participate in a video conference.
